Children and Student Ministry Volunteer Application

This document is designed to collect information about you that we need to qualify you for service in children's ministry at Fairview Baptist Church according to the Chilldren's and Student Ministry Policies and Procedures. This information is treated as confidential. Persons who review, duplicate, distribute, or disclose any portion of this document without authorization may face one or more of the following consequences: (1) Termination of employment, if an employee. (2) Dismissal from any official position within the church, if a volunteer. (3) Possible criminal liability. (4) Possible civil liability.

If at any time you determine that the information you provided in this document is not true--or if it changes in a way that substantively impacts your qualification to serve in childcare, you are requested to notify the pastor for discipleship and administration as soon as possible.

Consent: By entering your name, you consent to submit your application and Background Check Disclosure and Authorization Form and all related forms, documents and information electronically. You further consent to conduct any matters related to the recruiting, application, background check and/or onboarding process electronically. Children's/Youth Worker Verification and Release

I recognize that Fairview Baptist Church is relying on the accuracy of the information I provide on the Children/Youth Ministry Volunteer Application form. Accordingly, I attest and affirm that the information I have provided is absolutely true and correct.

I authorize Fairview Baptist Church to contact any person or entity listed on the Children/Youth Ministry Volunteer Application form, and I further authorize any such person or entity to provide Fairview Baptist Church with information, opinions, and impressions relating to my background or qualifications.

I voluntarily release Fairview Baptist Church and any such person or entity listed on the Children/Youth Ministry Volunteer Application form from liability involving the communication of information relating to my background or qualifications.
